Drug intelligence / Profile preview
Dual Antiplatelet Therapy + normal saline is a combination regimen used as the control arm in the TEARIAS (Tirofiban Enhanced Antiplatelet Therapy in Ruptured Intracranial Aneurysms With Stent) Phase 3 clinical trial. Sponsored by the First Affiliated Hospital of Fujian Medical University, this regimen consists of conventional dual antiplatelet therapy (DAPT)—typically comprising the small-molecule antiplatelet agents aspirin and clopidogrel—administered perioperatively, combined with an intravenous infusion of normal saline as a placebo comparator. The regimen is evaluated in patients with ruptured intracranial aneurysms accompanied by subarachnoid hemorrhage who are undergoing stent-assisted coiling. Aspirin acts by irreversibly inhibiting cyclooxygenase-1 (COX-1), thereby preventing thromboxane A2 synthesis, while clopidogrel selectively inhibits the binding of adenosine diphosphate (ADP) to its platelet P2Y12 receptor, both of which work synergistically to prevent platelet aggregation and stent thrombosis.
