Dual Antithrombotc Therapy-The First Affiliated Hospital with Nanjing Medical University-acute coronary syndrome-atrial fibrillation

Dual antithrombotic therapy (DAT) as defined in the OPTIMA-4 sub-study (sponsored by The First Affiliated Hospital with Nanjing Medical University) consists of the direct oral anticoagulant dabigatran etexilate combined with a P2Y12 inhibitor (either clopidogrel or ticagrelor). This combination is specifically investigated for patients with acute coronary syndrome (ACS) and non-valvular atrial fibrillation (NVAF) who have undergone percutaneous coronary intervention (PCI) with drug-eluting stents. Dabigatran acts as a direct thrombin inhibitor, while clopidogrel and ticagrelor inhibit platelet aggregation by antagonizing the P2Y12 receptor. The regimen aims to provide sufficient anticoagulation and antiplatelet effects to prevent stroke and stent thrombosis while minimizing the bleeding risks associated with triple antithrombotic therapy (which includes aspirin).
