DUALGRAFT is an investigational engineered hematopoietic stem cell transplant (HSCT) graft developed by Francisco Hernández Oliveros. It is designed to induce hematopoietic mixed chimerism in recipients of solid organ transplants (such as kidney, lung, or multivisceral transplants) to promote long-term immune tolerance and reduce the need for lifelong immunosuppression. The graft is specifically engineered to be enriched with CD34+ progenitor cells and memory T-lymphocytes (CD45RO+), while being depleted of naïve T-lymphocytes (CD45RA+). This composition aims to provide immune reconstitution and protection against infections via memory T cells while minimizing the risk of graft-versus-host disease (GvHD) typically associated with naïve T cells. The procedure involves a low-intensity conditioning regimen including total lymphoid irradiation and fludarabine prior to the infusion of the engineered graft.
