Dulaglutide + insulin glargine is a combination therapy consisting of **dulaglutide**, a glucagon-like peptide-1 (GLP-1) receptor agonist, and **insulin glargine**, a long-acting basal insulin analog, used in the treatment of type 2 diabetes mellitus. Dulaglutide acts by binding to the GLP-1 receptor, enhancing glucose-dependent insulin secretion, suppressing inappropriate glucagon secretion, slowing gastric emptying, and reducing food intake. Insulin glargine provides long-lasting, steady glucose-lowering activity by stimulating peripheral glucose uptake (especially in skeletal muscle and fat) and inhibiting hepatic glucose production. The combination is used in patients requiring escalation of injectable therapy for improved glycemic control. The AWARD-9 trial established the efficacy and safety of adding dulaglutide to insulin glargine, resulting in greater HbA1c reduction and modest weight loss compared to insulin alone, with a lower requirement for total insulin dose and without increased risk of severe hypoglycemia[5][7].
