Duligotuzumab (MEHD7945A) is a novel, dual-action humanized IgG1 monoclonal antibody designed to simultaneously target and inhibit the epidermal growth factor receptor (EGFR) and the human epidermal growth factor receptor 3 (HER3). Developed by Genentech, it utilizes a "two-in-one" approach where each antigen-binding fragment (Fab) can bind to either receptor, thereby blocking ligand-induced signaling from various HER family dimers (EGFR/EGFR, EGFR/HER2, EGFR/HER3, and HER2/HER3). Additionally, as an IgG1 antibody, it can elicit antibody-dependent cell-mediated cytotoxicity (ADCC). Despite promising preclinical data suggesting it could overcome resistance to EGFR-only inhibitors, clinical trials in metastatic colorectal cancer (mCRC) and squamous cell carcinoma of the head and neck (SCCHN) failed to demonstrate superior efficacy compared to cetuximab. Consequently, its clinical development has been discontinued.
