duoCAR-T is an autologous anti-HIV CAR-T cell therapy being developed by Caring Cross for HIV infection and AIDS. The product uses patient-derived T cells genetically modified with an HIV-1-based lentiviral vector to express a two-molecule chimeric antigen receptor architecture directed against multiple highly conserved epitopes on the HIV-1 envelope glycoprotein gp120, including the CD4 binding site and co-receptor binding site, while also incorporating the C46 fusion-inhibitory peptide to protect the engineered cells from HIV entry. The therapeutic intent is to create HIV-resistant effector T cells that can recognize and eliminate HIV-infected cells; preclinical reports described potent killing of infected cells and strong viral suppression in humanized mouse models, and the program has been in early clinical development for HIV/AIDS.
