This combination therapy consists of the immune checkpoint inhibitor durvalumab and the cytotoxic chemotherapy agents doxorubicin and ifosfamide. It is being investigated primarily by Seoul National University Hospital for the treatment of pulmonary sarcomatoid carcinoma (PSC), a rare and aggressive subtype of non-small cell lung cancer. Durvalumab is a human monoclonal antibody that blocks the interaction of programmed cell death ligand 1 (PD-L1) with PD-1 and CD80, thereby enhancing T-cell anti-tumor activity. Doxorubicin acts by intercalating DNA and inhibiting topoisomerase II, while ifosfamide is an alkylating agent that causes DNA cross-linking. The regimen typically involves an induction phase of the triplet combination followed by durvalumab monotherapy maintenance.
