A combination therapy regimen consisting of the PD-L1 inhibitor durvalumab, the chemotherapy agents gemcitabine and cisplatin, and yttrium-90 (Y-90) radioembolization. This specific protocol is being investigated by Inova Health Care Services for the treatment of locally advanced unresectable intrahepatic cholangiocarcinoma (iCCA). Durvalumab is a human monoclonal antibody that blocks the interaction of programmed death-ligand 1 (PD-L1) with PD-1 and CD80, thereby restoring the anti-tumor activity of cytotoxic T cells. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by targeting ribonucleotide reductase, while cisplatin is a platinum-based chemotherapy that forms DNA cross-links, leading to apoptosis. Y-90 radioembolization involves the intra-arterial delivery of microspheres containing the beta-emitting isotope yttrium-90, providing targeted internal radiation to hepatic tumors. The treatment sequence includes induction with the chemo-immunotherapy triplet, followed by concurrent Y-90, and subsequent consolidation therapy.
