Durvalumab + lenalidomide is a pharmaceutical combination of two agents: **durvalumab**, a human monoclonal antibody that targets programmed death-ligand 1 (PD-L1), and **lenalidomide**, an oral immunomodulatory drug. Durvalumab exerts its effect by blocking PD-L1 on tumor and immune cells, thereby restoring and enhancing anti-tumor T-cell immune response. Lenalidomide acts primarily by modulating the immune system, activating T cells, inhibiting pro-inflammatory cytokines, and reducing regulatory immune cell populations in the tumor microenvironment. This combination is under investigation primarily for refractory or advanced **cutaneous T-cell lymphoma (CTCL)**, including mycosis fungoides and Sézary syndrome, where it has shown superior clinical activity compared to durvalumab monotherapy, with durable responses and acceptable safety in early-phase clinical trials[1][2][4][5][6].
