This is an experimental **combination therapy** consisting of three agents: **durvalumab** (a monoclonal antibody that targets programmed death ligand 1, PD-L1), **lenalidomide** (an immunomodulatory and anti-neoplastic small molecule agent), and **dexamethasone** (a synthetic glucocorticoid corticosteroid). It has been studied in a Phase 1/2 clinical trial for the treatment of newly diagnosed multiple myeloma, particularly in transplant-ineligible and/or high-risk patients. The rationale stems from potential enhancement of anti-myeloma immune responses by combining immunomodulation (lenalidomide) with checkpoint inhibition (durvalumab) and corticosteroid-mediated anti-inflammatory/antitumor activity (dexamethasone). The main study (MEDI4736-MM-002) was placed on full clinical hold and terminated early by the FDA due to safety concerns extrapolated from similar combinations involving PD-1 inhibitors and immunomodulators in multiple myeloma[1][3][5][6][7].
