Overview

This combination therapy, referred to as the HAIC-quad regimen, is being investigated by West China Hospital as a first-line treatment for locally advanced or metastatic intrahepatic cholangiocarcinoma (ICC). It consists of hepatic arterial infusion chemotherapy (HAIC) using gemcitabine and cisplatin, combined with the immune checkpoint inhibitor durvalumab and the multikinase inhibitor lenvatinib. HAIC allows for high local concentrations of chemotherapeutic agents in the liver while minimizing systemic toxicity. Durvalumab targets the PD-L1 pathway to enhance anti-tumor immune responses, while lenvatinib inhibits multiple receptor tyrosine kinases involved in angiogenesis and tumor progression, including VEGFR and FGFR.
