Overview

Durvalumab + polyICLC is an investigational combination therapy consisting of two agents: - **Durvalumab** is a human monoclonal antibody targeting programmed death-ligand 1 (PD-L1), functioning as an immune checkpoint inhibitor. By blocking PD-L1, it prevents the inhibition of T-cell activation and enhances antitumor immune responses. - **PolyICLC** (polyinosinic-polycytidylic acid stabilized with poly-L-lysine and carboxymethylcellulose) is a synthetic double-stranded RNA that acts as an agonist for toll-like receptor 3 (TLR3) and melanoma differentiation-associated protein 5 (MDA5). It stimulates innate immunity by inducing type I interferons and other cytokines. The combination aims to enhance both innate and adaptive antitumor immunity. Clinical trials have evaluated this regimen in advanced solid tumors such as breast cancer, where intratumoral administration of polyICLC combined with systemic intravenous durvalumab has shown safety and preliminary clinical activity[1][9]. The approach seeks to improve tumor infiltration by immune cells through local stimulation while simultaneously relieving immunosuppression via checkpoint blockade.
