This therapeutic regimen, studied by Hamilton Health Sciences Corporation in the DARTS trial, consists of the combination of two immune checkpoint inhibitors—durvalumab and tremelimumab—administered alongside stereotactic body radiation therapy (SBRT) for the treatment of resectable hepatocellular carcinoma (HCC). Durvalumab is a human monoclonal antibody that targets programmed cell death ligand 1 (PD-L1), while tremelimumab is a human monoclonal antibody targeting cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). The addition of SBRT is hypothesized to induce immunogenic cell death and alter the tumor microenvironment, potentially increasing the infiltration of effector CD8+ T-cells and reducing immunosuppressive cells like regulatory T cells (Tregs) and M2 macrophages. This multimodal approach aims to improve pathological and biological responses prior to surgical resection.
