**Duvelisib + methotrexate** is an investigational combination therapy consisting of duvelisib, a small molecule inhibitor of phosphoinositide 3-kinase delta (PI3Kδ) and gamma (PI3Kγ), and methotrexate, a folate antagonist that inhibits nucleotide synthesis. The combination has been clinically evaluated in moderate-to-severe rheumatoid arthritis, where duvelisib was administered alongside a stable dose of methotrexate. Duvelisib is approved as Copiktra for certain hematological malignancies. In the RA trial, the combination did not meet the primary efficacy endpoint and showed no large or consistent improvements compared to placebo plus methotrexate, although it was generally well tolerated.
