DW4421-1 is a capsule formulation of padoprazan, a small molecule potassium-competitive acid blocker (P-CAB) developed by Daewon Pharmaceutical in South Korea. It functions by competitively and reversibly inhibiting the H+/K+-ATPase enzyme (the proton pump) in gastric parietal cells, thereby suppressing gastric acid secretion. Unlike conventional proton pump inhibitors (PPIs), P-CABs do not require acid activation and provide a faster onset of action with pH-independent inhibition. DW4421-1 is specifically being evaluated in Phase 1 clinical trials for the treatment of Helicobacter pylori infection, particularly in drug-drug interaction studies to assess its safety and pharmacokinetics when co-administered with standard antibiotics such as amoxicillin and clarithromycin. The active moiety, padoprazan, is also in Phase 3 development for the treatment of gastroesophageal reflux disease (GERD).
