The drug acts as a neuromuscular blocking agent by entering peripheral cholinergic nerve terminals and enzymatically cleaving the synaptosomal-associated protein 25 (SNAP-25). This cleavage prevents the formation of the SNARE complex, thereby inhibiting the exocytosis of acetylcholine into the neuromuscular junction, resulting in temporary muscle relaxation and the smoothing of wrinkles.
