Ebselen is a synthetic organoselenium compound with anti-inflammatory, antioxidant, and cytoprotective properties. It acts primarily as a glutathione peroxidase mimetic, reducing cellular damage from reactive oxygen species (ROS) and peroxynitrite. Ebselen also exhibits inhibitory activity against inositol monophosphatase (IMPase), cyclooxygenase 2 (COX2), leukotriene B4 receptor, prostaglandin receptors, and certain viral proteases. Its mechanisms make it of interest for neuroprotection and the treatment of conditions involving oxidative stress or inflammation. Ebselen has been investigated for Meniere’s disease (with positive Phase III results), bipolar disorder/mania, hearing loss (including noise-induced and sensorineural types), tinnitus, COVID infections, stroke recovery, depression, diabetes mellitus type 1 and type 2, among others. The most advanced clinical development is for Meniere’s disease as an oral capsule formulation called SPI‑1005[1][3][4][5][8].
