ECCE-expanded tumor-infiltrating lymphocytes (TILs) are an autologous adoptive cell therapy developed by the National Cancer Institute (NCI) for the treatment of metastatic melanoma. This specific approach involves harvesting TILs from a patient's tumor tissue and expanding them ex vivo using engineered cells for costimulation enhancement (ECCE). These engineered cells, typically K562 cells modified to express costimulatory ligands such as 4-1BBL and membrane-bound cytokines like IL-15, are used to improve the activation, persistence, and anti-tumor potency of the T cells. The therapy is administered as a single intravenous infusion following a lymphodepleting preparative regimen (usually cyclophosphamide and fludarabine) and is followed by high-dose aldesleukin (IL-2) to support the expansion and survival of the infused lymphocytes in vivo.
