Echothiophate chloride is a long-acting organophosphate acetylcholinesterase inhibitor. While echothiophate (typically as the iodide salt) has historically been used as an ophthalmic solution for the treatment of glaucoma, the chloride salt is being developed as an injectable formulation for the reversal of residual neuromuscular blockade following general anesthesia. It works by forming a stable, covalent bond with the enzyme acetylcholinesterase, preventing the hydrolysis of acetylcholine. This leads to an accumulation of acetylcholine at the nicotinic receptors of the neuromuscular junction, which antagonizes the effects of non-depolarizing neuromuscular blocking agents. In clinical settings, it is typically administered alongside an anticholinergic agent, such as atropine, to prevent systemic muscarinic effects like bradycardia and excessive secretions. Jinan Bosch Pharmaceutical Technology is investigating this formulation in Phase 1 clinical trials.
