Edecesertib (formerly known as GS-5718) is an orally bioavailable, potent, and selective small molecule inhibitor of interleukin-1 receptor-associated kinase 4 (IRAK4). Developed by Gilead Sciences, it targets the IRAK4 protein, which is a key mediator in the signaling pathways of Toll-like receptors (TLRs) and interleukin-1 receptors (IL-1Rs), which are involved in the production of pro-inflammatory cytokines. The drug is currently being evaluated in the Phase 2a COSMIC trial for the treatment of cutaneous lupus erythematosus (CLE). Although it was previously investigated for other inflammatory diseases such as rheumatoid arthritis, Gilead has narrowed its focus to lupus-related indications, with some earlier programs reported as discontinued.
