Overview

Efavirenz + nevirapine is a non-standard, off-label combination of two non-nucleoside reverse transcriptase inhibitors (NNRTIs) used in antiretroviral therapy for HIV infection. Both drugs act by inhibiting the reverse transcriptase enzyme of HIV, thereby preventing viral replication. Efavirenz is known for its central nervous system side effects and teratogenicity risk, while nevirapine carries risks of hepatotoxicity and severe rash. Standard first-line regimens typically use one NNRTI plus two nucleoside reverse transcriptase inhibitors (NRTIs), not both NNRTIs together; thus, this dual-NNRTI combination is not recommended or approved in major guidelines due to lack of evidence for added benefit and increased risk of toxicity[1][3]. Both drugs are widely used individually as part of three-drug regimens with NRTIs.
