MEDI1873 (efgivanermin alfa) is a novel, potent, stabilized hexameric agonist of the human glucocorticoid-induced tumor necrosis factor receptor-related protein (GITR). It is a recombinant fusion protein composed of an IgG1 Fc domain, a coronin 1A trimerization domain, and the extracellular domain of human GITRL. MEDI1873 acts as an agonist at GITR, enhancing effector T-cell activation and inhibiting regulatory T-cell suppression. This mechanism aims to promote antitumor immunity by increasing CD8+ T cell responses and modulating immune regulation within the tumor microenvironment. Developed for use in advanced solid tumors, it demonstrated acceptable safety and pharmacodynamic activity in early-phase clinical trials but did not show sufficient efficacy to warrant further development[1][2][5][7].
