Overview

Efpegerglucagon is a long-acting glucagon analog developed by Hanmi Pharmaceutical for the treatment of chronic hypoglycemic diseases, particularly congenital hyperinsulinism (CHI) and hypoglycemia post-bariatric surgery. It is chemically conjugated with the constant region of human immunoglobulin via a non-peptidyl flexible linker, resulting in favorable pharmacokinetic properties that support once-weekly dosing. Efpegerglucagon acts as a glucagon receptor agonist, increasing glycogenolysis and gluconeogenesis to counteract excessive insulin activity and stabilize blood glucose levels. The drug has received orphan drug designation from regulatory agencies including the FDA, EMA, and MFDS for CHI and rare pediatric disease designation from the US FDA. Clinical trials have demonstrated significant reductions in hypoglycemia events with good safety and tolerability profiles[1][2][3][4][5].
