**Efpeglenatide + insulin** (also known as LAPSInsulin Combo) is an investigational once-weekly combination therapy for diabetes comprising efpeglenatide, a long-acting glucagon-like peptide-1 receptor agonist (GLP-1RA) based on a modified exendin-4 analog fused to an IgG4 Fc fragment via LAPS technology for prolonged half-life, and LAPSInsulin 115, a long-acting insulin analog engineered for weekly dosing using Hanmi Pharm's LAPSCOVERY platform. Efpeglenatide acts as a potent GLP-1RA agonist promoting insulin secretion, glucagon suppression, gastric emptying delay, and satiety for glycemic control and weight loss, while the insulin component provides basal glycemic regulation. Developed by Hanmi Pharm, this first-in-class fixed-dose combination aims to simplify injectable diabetes management by reducing injection frequency versus daily insulins and daily/weekly GLP-1RAs alone, targeting type 2 diabetes with potential for improved adherence.[5]
