Overview

Eftozanermin (ABBV-621) is a first-in-class, hexavalent TRAIL (TNF-related apoptosis-inducing ligand) receptor agonist developed by AbbVie. It is a recombinant fusion protein consisting of a human IgG1 Fc domain linked to two sets of stabilized TRAIL trimer subunits. This unique hexavalent structure is designed to overcome the limitations of earlier TRAIL-based therapies by enabling potent clustering of Death Receptor 4 (DR4) and Death Receptor 5 (DR5) on the surface of cancer cells. This clustering triggers the extrinsic apoptotic pathway, leading to programmed cell death in susceptible tumor cells. Eftozanermin is primarily being investigated for the treatment of hematologic malignancies, such as relapsed or refractory multiple myeloma and acute myeloid leukemia, as well as various solid tumors, often in combination with other targeted agents like venetoclax.
