Efzilonkofusp alfa (GI-102) is an investigational bispecific Fc fusion protein (immunocytokine) developed by GI Innovation, Inc. It is designed to target and deliver a mutated interleukin-2 variant (IL-2v3) specifically to immune and tumor cells. Structurally, it consists of the N-terminal extracellular domain of human CD80 (B7-1) fused to a human IgG4 Fc fragment, which is linked to the IL-2v3 at the C-terminus. The CD80 portion acts as an anchoring domain that binds to PD-L1 and blocks CTLA-4, while the IL-2v3 portion has abolished affinity for IL-2Rα (CD25) to minimize regulatory T cell (Treg) activation, while retaining high affinity for IL-2Rβγ to selectively expand and activate CD8+ T cells and natural killer (NK) cells. Efzilonkofusp alfa is being evaluated in clinical trials for advanced or metastatic solid tumors (such as sarcoma and melanoma) and, at low doses, in combination with the synbiotic GIB-7 for anti-aging applications to reverse immunosenescence.
