Overview

Eganelisib is a first-in-class, orally administered, highly selective PI3K-gamma inhibitor developed for cancer therapy. It acts by inhibiting the PI3K-gamma pathway, which plays a role in tumor growth and immune suppression within the tumor microenvironment. Cytarabine is an antimetabolite chemotherapy agent that interferes with DNA synthesis, primarily used to treat various forms of leukemia including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), and chronic myeloid leukemia (CML). The combination of eganelisib and cytarabine is being investigated for relapsed/refractory AML and higher-risk myelodysplastic syndromes (HR-MDS). Preclinical studies suggest that this combination works synergistically: eganelisib suppresses oxidative phosphorylation in leukemia cells—an energy pathway critical for their survival—while cytarabine disrupts DNA synthesis. This dual mechanism may help overcome resistance to standard therapies and target residual disease after initial treatment[1][2][5].
