EGEN-5784 is a human-compatible, genetically engineered porcine liver developed for use as an extracorporeal organ support system in patients with acute-on-chronic liver failure (ACLF) who are not eligible for transplant. The organ is used in combination with the OrganOx extracorporeal liver cross-circulation (ELC) system to temporarily support or replace native liver function, potentially allowing time for the patient’s own liver to recover or until a suitable donor organ becomes available. The genetic modifications of EGEN-5784 include knockout of three genes involved in glycan antigen synthesis (to prevent hyperacute rejection), insertion of seven human transgenes that regulate inflammation, innate immunity, coagulation, and complement pathways (to modulate immune response and compatibility), and inactivation of endogenous retroviruses within the porcine genome. This approach represents a novel application of xenotransplantation technology aimed at addressing critical unmet needs in end-stage liver disease[1][3][5][7][8].
