EGFL6 monoclonal antibody is a humanized, affinity-matured therapeutic antibody designed to neutralize epidermal growth factor-like domain multiple-6 (EGFL6). EGFL6 is a secreted protein overexpressed in several malignancies, including high-grade serous ovarian cancer, breast cancer, and colorectal cancer, where it is produced by both tumor cells and cancer-associated fibroblasts. The antibody works by blocking EGFL6-mediated signaling, which otherwise promotes tumor angiogenesis, cancer cell proliferation, and metastasis. Crucially, recent research has identified EGFL6 as a regulator of tumor immunogenicity; it induces the internalization and lysosomal degradation of Major Histocompatibility Complex class I (MHC-I) molecules. By inhibiting EGFL6, the monoclonal antibody restores surface MHC-I stability, thereby enhancing antigen presentation and increasing the infiltration and activity of CD8+ T cells within the tumor microenvironment. This dual action—inhibiting tumor growth/angiogenesis and reprogramming the immune environment—provides a strong rationale for its use as a monotherapy or in combination with immune checkpoint inhibitors. The antibody is currently in preclinical development, with plans for first-in-human clinical trials specifically targeting ovarian cancer.
