EGFR-41BB-CD3zeta CAR T is a second-generation chimeric antigen receptor (CAR) T-cell therapy designed to target the epidermal growth factor receptor (EGFR) and its variants, most notably EGFRvIII, which are frequently overexpressed in glioblastoma (GBM). The therapeutic construct consists of an extracellular single-chain variable fragment (scFv) specific for EGFR/EGFRvIII, linked to an intracellular 4-1BB (CD137) costimulatory domain and a CD3ζ signaling domain. Upon binding to the target antigen on tumor cells, the CAR triggers T-cell activation, proliferation, and cytotoxic activity. Clinical investigations have demonstrated that while this construct can induce rapid, early tumor reductions in GBM, the responses are often transient due to T-cell exhaustion or antigen escape. It is currently utilized both as a clinical candidate and as a benchmark for evaluating next-generation CAR architectures, such as KIR-CAR T cells.
