EGFR806 KIR CAR T cells are an investigational autologous chimeric antigen receptor (CAR) T-cell therapy being developed for the treatment of glioblastoma (GBM). The therapy utilizes a CAR construct featuring a single-chain variable fragment (scFv) derived from the monoclonal antibody 806 (mAb806). This binder specifically targets a conformationally exposed epitope of the epidermal growth factor receptor (EGFR) that is accessible on EGFRvIII, amplified wild-type EGFR, and certain EGFR extracellular domain missense mutations (e.g., R108K), but remains hidden on normal wild-type EGFR in healthy tissues, thereby reducing off-tumor toxicity. Uniquely, this construct employs a Killer Immunoglobulin-like Receptor (KIR) co-stimulatory domain rather than traditional 4-1BB or CD28 domains. Preclinical studies suggest that the KIR-based architecture enhances anti-tumor efficacy and may reduce the risk of graft-versus-host disease (GVHD) compared to standard CAR designs.
