Elacestrant + capivasertib is an investigational, all-oral combination of two small molecule agents under evaluation for the treatment of **estrogen receptor positive (ER+)/HER2-negative locally advanced or metastatic breast cancer**, particularly in hormone receptor–positive patients who have progressed on prior endocrine therapy and CDK4/6 inhibitors[1][5][6]. **Elacestrant** is a selective estrogen receptor degrader (SERD) that promotes degradation of the estrogen receptor, thereby blocking estrogen signaling that drives growth in ER+ breast cancers. **Capivasertib** is a pan-AKT inhibitor that targets AKT (protein kinase B), inhibiting a key node of the PI3K/AKT pathway frequently activated in resistant breast cancers[2][6]. The combination aims to overcome resistance that develops after standard endocrine therapy by dual targeting: elacestrant blocks ER signaling, while capivasertib suppresses compensatory activation in the PI3K/AKT pathway. This strategy is being evaluated in the phase 1b/2 ELEVATE trial and other studies[1][5][6].
