Elarekibep is a first-in-class inhaled Anticalin protein therapeutic engineered from human lipocalin 1 (Lcn1) to act as a high-affinity, long-acting full antagonist of interleukin-4 receptor alpha (IL-4Ra). It competitively blocks IL-4 and IL-13 signaling through IL-4Ra, key drivers of type 2 endotype asthma. Elarekibep was designed for direct delivery to the lungs via inhalation, aiming to provide local efficacy with reduced systemic exposure compared to injectable biologics like dupilumab. The drug was developed using directed protein mutagenesis and demonstrated potent inhibition of IL-4/IL-13-induced STAT6 phosphorylation and suppression of allergic inflammation in preclinical models. Its clinical development targeted moderate-to-severe asthma but was discontinued after non-clinical toxicology studies revealed lung tissue damage in primates[1][3][4][6][9].
