ELGN-GI is a proprietary oral formulation of recombinant human insulin developed specifically for neonatal use. It is administered enterally (orally or via tube) and acts locally within the gastrointestinal (GI) tract to accelerate intestinal development in preterm infants. Unlike systemic insulin therapies, ELGN-GI does not enter the bloodstream, limiting its effects to the GI tract. The drug mimics natural exposure to insulin that would occur during a full-term pregnancy, promoting faster maturation of the intestines and enabling infants to achieve nutritional independence more rapidly. Mechanistically, it functions as an ornithine decarboxylase stimulant, phosphokinase stimulant, and protein tyrosine kinase stimulant. Its primary indications are for treating intestinal malabsorption and feeding intolerance in preterm infants—conditions often associated with short bowel syndrome and other causes of intestinal failure[1][2][4][5].
