ELNVM refers to exosome-like nanovesicles (ELNVs) derived from unpretreated mesenchymal stem cells (MSCs). These nanovesicles are being investigated for their potential to treat heart failure with preserved ejection fraction (HFpEF) by targeting visceral adipose tissue (VAT) inflammation. ELNVMs promote an anti-inflammatory phenotype switch in macrophages, reducing the expression of pro-inflammatory markers such as inducible nitric oxide synthase (iNOS) and interleukin-1β (IL-1β), while increasing the expression of anti-inflammatory markers like Arginase-1 (Arg1). In preclinical models of HFpEF, ELNVM treatment has been shown to reduce cardiac hypertrophy, pulmonary congestion, and improve diastolic function (E/E').
