Overview

ELNVMA (also referred to as ELN-vma) is an experimental exosome-like nanovesicle (ELNV) derived from mesenchymal stem cells (MSCs) that have been pre-conditioned with atorvastatin. Developed by researchers at Fuwai Hospital, this therapeutic candidate is designed to address heart failure with preserved ejection fraction (HFpEF) by targeting chronic inflammation in visceral adipose tissue (VAT). The mechanism of action involves the delivery of specific long non-coding RNAs (lncRNAs)—namely TARID, H19, and KLF3-AS1—which modulate the local immune environment by promoting a phenotypic switch in macrophages from a pro-inflammatory M1 state to an anti-inflammatory M2 state. Preclinical studies in obese mouse models have shown that ELNVMA treatment reduces VAT inflammation (lowering IL-1β and iNOS levels), attenuates cardiac hypertrophy, and improves diastolic function, suggesting a potential role for MSC-derived nanovesicles in treating obesity-related cardiovascular diseases.
