Elocalcitol is a synthetic vitamin D3 (calcitriol) analog developed primarily for the treatment of benign prostatic hyperplasia (BPH), overactive bladder, prostatitis, and male infertility. It acts as an agonist of the vitamin D receptor (VDR), regulating cell proliferation and apoptosis. Elocalcitol exhibits anti-proliferative and anti-inflammatory effects by inhibiting interleukin 8 (IL-8) production, suppressing RhoA/Rho kinase signaling pathways, and modulating L-type calcium channel activity in smooth muscle cells. In clinical trials, it demonstrated efficacy in reducing prostate volume in BPH patients and improving sperm quality in prostatitis but failed to meet primary endpoints for overactive bladder. All further development was discontinued after phase II trials due to insufficient efficacy[1][2][4][5][7].
