Overview

Eloralintide (LY3841136) is a novel, long-acting, selective amylin receptor agonist developed by Eli Lilly for the treatment of obesity and overweight. It is a peptide analog of amylin modified with a C20 fatty diacid moiety to extend its plasma half-life, enabling once-weekly subcutaneous administration. Eloralintide is designed to preferentially activate amylin receptors (specifically AMY1R and AMY3R) while retaining selectivity over the calcitonin receptor (CTR). This pharmacological profile is intended to promote significant weight loss primarily through fat mass reduction while improving gastrointestinal tolerability and minimizing nausea compared to non-selective amylin analogs. Clinical data from Phase 2 trials have demonstrated weight loss of up to 20% over 48 weeks, positioning it as a potential best-in-class therapy both as a monotherapy and in combination with GLP-1 receptor agonists like tirzepatide.
