Elotuzumab + selinexor + dexamethasone is a combination regimen under investigation for the treatment of relapsed or refractory multiple myeloma. Elotuzumab is a monoclonal antibody that targets SLAMF7 (also known as CS1) on myeloma and natural killer (NK) cells, activating NK cells and tagging myeloma cells for immune-mediated destruction. Selinexor is an orally bioavailable small molecule that selectively inhibits exportin 1 (XPO1), blocking nuclear export of tumor suppressor proteins and other regulatory proteins, leading to cancer cell death. Dexamethasone is a synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressive properties, commonly used in multiple myeloma regimens to enhance antitumor effects[3][6][7]. This combination leverages complementary mechanisms—immune activation, inhibition of oncogenic protein transport, and anti-inflammatory action—to target resistant or relapsed disease.
