Elraglusib + retifanlimab + mFOLFIRINOX is an investigational combination regimen being evaluated as a frontline therapy for patients with advanced pancreatic adenocarcinoma. This regimen is the focus of the Phase 1b RiLEY trial (NCT06896188), an investigator-initiated study led by Dr. Anwaar Saeed at UPMC Hillman Cancer Center. The combination includes elraglusib (9-ING-41), a small molecule inhibitor of glycogen synthase kinase-3 beta (GSK-3β) developed by Actuate Therapeutics; retifanlimab, an anti-PD-1 monoclonal antibody developed by Incyte Corporation; and modified FOLFIRINOX (mFOLFIRINOX), a standard chemotherapy regimen consisting of oxaliplatin, irinotecan, leucovorin, and 5-fluorouracil. The rationale for this combination is to leverage the immunomodulatory and chemo-sensitizing effects of GSK-3β inhibition alongside immune checkpoint blockade and potent cytotoxic chemotherapy to improve outcomes in a traditionally treatment-resistant disease.
