Elsulfavirine + nucleoside reverse transcriptase inhibitor + integrase strand transfer inhibitor is a fixed-dose combination (FDC) therapy under development by Viriom and Ritverc for the treatment and prophylaxis of HIV-1 infection. The regimen centers on elsulfavirine (VM-1500), a long-acting non-nucleoside reverse transcriptase inhibitor (NNRTI) that acts as a prodrug for the active moiety VM-1500A. By combining elsulfavirine with a nucleoside reverse transcriptase inhibitor (NRTI) and an integrase strand transfer inhibitor (INSTI), the therapy targets two critical enzymes in the viral replication cycle: reverse transcriptase and integrase. This multi-target approach is designed to increase the genetic barrier to resistance and provide a complete, simplified dosing regimen. The combination is currently in Phase 3 clinical development, aiming to provide a robust option for both treatment-naive patients and those requiring pre-exposure prophylaxis (PrEP).
