EM939 is a 12-membered non-antibiotic macrolide derivative developed as an anti-inflammatory and immunomodulatory agent. It is a derivative of EM900, which is itself derived from erythromycin A. Unlike traditional macrolides, EM939 is engineered to lack antibacterial activity, which avoids the risk of promoting bacterial resistance during long-term therapy. Its primary biological effect is the promotion of monocyte-to-macrophage differentiation, a marker for its immunomodulatory properties. In preclinical cigarette-smoking models used to simulate chronic obstructive pulmonary disease (COPD), EM939 has demonstrated the ability to significantly attenuate lung and alveolar inflation functionally and histologically when administered orally. This suggests potential therapeutic utility in treating chronic inflammatory respiratory diseases.
