Overview

Emapunil (AC5216, also known as XBD-173) is a small molecule drug that acts as a selective agonist at the translocator protein (TSPO), formerly known as the peripheral benzodiazepine receptor. TSPO is located primarily on the outer mitochondrial membrane and plays a key role in steroidogenesis, including the production of neuroactive steroids such as allopregnanolone in the brain. Emapunil has demonstrated anxiolytic and antidepressant-like effects in animal models and human studies without causing sedation or withdrawal symptoms typical of classical benzodiazepines. It has been investigated for use in generalized anxiety disorder, major depressive disorder, and post-traumatic stress disorder (PTSD). Additionally, radiolabeled forms of emapunil have been used for PET imaging to map TSPO distribution in the brain[1][2][5][6][7].
