Overview

Embryonic mesenchymal stem cells (EMSCs) are multipotent stromal cells derived from human embryonic stem cells (hESCs) through directed differentiation. These cells represent a standardized alternative to adult-derived mesenchymal stem cells (MSCs), such as those from bone marrow or adipose tissue, potentially offering superior scalability and phenotypic consistency. In clinical applications for refractory systemic sclerosis, EMSCs are utilized for their potent immunomodulatory and anti-fibrotic properties. They function by secreting a variety of paracrine factors—including cytokines, chemokines, and extracellular vesicles—that suppress the activation of inflammatory T-cells and modulate the activity of myofibroblasts, thereby addressing the underlying autoimmune dysfunction and excessive tissue fibrosis characteristic of the disease. The Affiliated Hospital of Xuzhou Medical University has initiated clinical investigations (e.g., ChiCTR1800018610) to evaluate the safety and therapeutic potential of these cells in patients who have failed standard immunosuppressive therapies.
