Overview

**empagliflozin + ramipril** is a combination of two small molecule drugs: empagliflozin, a sodium-glucose cotransporter 2 inhibitor (SGLT2 inhibitor), and ramipril, an angiotensin-converting enzyme inhibitor (ACE inhibitor). This combination targets both glycemic control and cardiovascular/renal risk in patients with diabetes and/or cardiovascular disease. Empagliflozin works by inhibiting SGLT2 in the proximal tubules of the kidney, reducing glucose reabsorption and promoting its urinary excretion, thus lowering blood sugar and providing cardiorenal protection. Ramipril inhibits angiotensin-converting enzyme, reducing angiotensin II synthesis, thereby lowering blood pressure, decreasing total peripheral resistance, and providing additional cardiovascular and renal protective effects. The combination has been shown to cause an additive reduction in blood pressure, total peripheral resistance, glomerular filtration rate, and suppression of oxidative stress markers, consistent with a protective cardiorenal profile in patients with diabetes and elevated cardiovascular risk[1].
