Empiric TB treatment refers to the initial administration of a standard combination of first-line antituberculosis drugs before laboratory confirmation of Mycobacterium tuberculosis infection. This approach is used when there is a high clinical suspicion for active tuberculosis, especially in seriously ill patients or those at risk for rapid disease progression. The standard regimen consists of an intensive phase with four drugs—isoniazid (INH), rifampin (RIF), pyrazinamide (PZA), and ethambutol (EMB)—for two months, followed by a continuation phase with isoniazid and rifampin for four months. These agents act through different mechanisms to inhibit mycobacterial cell wall synthesis, nucleic acid synthesis, and energy metabolism. Empiric therapy aims to rapidly reduce bacterial load, prevent resistance development, minimize transmission risk, and ensure patient survival while awaiting diagnostic confirmation.
