During the second week (days 8-14), a third antibiotic—either levofloxacin (Cravit), clarithromycin (Klaricid), or tetracycline—is added based on the patient's prior medication history. The regimen is designed to overcome antibiotic resistance in refractory *H. pylori* cases by utilizing a high-dose PPI to stabilize antibiotics and a rotating combination of antimicrobial agents with distinct mechanisms of action, including cell wall synthesis inhibition, DNA damage, and protein synthesis inhibition.
