Emraclidine is an investigational small molecule antipsychotic developed by Cerevel Therapeutics for the treatment of schizophrenia and Alzheimer's disease psychosis. It acts as a highly selective positive allosteric modulator of the muscarinic acetylcholine receptor M4 subtype, which is primarily expressed in the striatum of the brain. By selectively targeting M4 receptors, emraclidine aims to regulate acetylcholine and dopamine levels indirectly, potentially reducing psychotic symptoms without blocking dopamine D2/D3 receptors—thereby minimizing motor side effects commonly seen with current antipsychotics. Emraclidine has been studied in multiple clinical trials; while early-phase studies showed promise, phase 2 trials did not demonstrate significant improvement over placebo on primary endpoints for schizophrenia[1][2][6][7][8].
