This is a combination regimen consisting of four active pharmaceutical ingredients: - **Emtricitabine** (a nucleoside reverse transcriptase inhibitor) and **tenofovir disoproxil fumarate** (a nucleotide reverse transcriptase inhibitor), commonly co-formulated as Truvada. These drugs inhibit HIV-1 reverse transcriptase by competing with natural substrates and causing DNA chain termination. - **Raltegravir** (MK-0518; brand name Isentress), an integrase strand transfer inhibitor that blocks the action of HIV integrase enzyme required for viral replication. - **Losartan**, an angiotensin II receptor blocker (ARB) used primarily to treat hypertension by blocking the binding of angiotensin II to the AT1 receptor in vascular smooth muscle. This combination would be used in a patient requiring antiretroviral therapy for HIV infection along with antihypertensive treatment. Emtricitabine/tenofovir is indicated for both treatment and pre-exposure prophylaxis (PrEP) against HIV; raltegravir is added as part of a complete antiretroviral regimen; losartan manages blood pressure[2][5][7].
