Emvistegrast is an oral small-molecule α4β7 integrin antagonist developed by Gilead Sciences for the treatment of inflammatory bowel disease, with a primary focus on ulcerative colitis. It is a quinolone-derived, once-daily, gut-selective agent that inhibits the α4β7 integrin–mucosal addressin cell adhesion molecule 1 (MAdCAM-1) interaction to reduce lymphocyte trafficking to the gut, targeting the same clinically validated pathway as the monoclonal antibody vedolizumab but in an orally bioavailable format.[1][3][5][6][7] Emvistegrast (GS-1427) is in phase 2 clinical development for moderately to severely active ulcerative colitis, including a randomized, double-blind, placebo-controlled, dose-ranging trial (SWIFT; NCT06290934).[4][6][7][9]
